Youth In Transition

The Youth In Transition program provides Life skills, workshops and, team building activities grounded in an Afrocentric framework for Black Youth from the community and/or involved with Children’s Aid Society/the welfar system..

The Team consists of a Youth In Transition Worker and a Housing Support Worker.

The objectives and focus of both roles are:

  • To improve the outcomes and increase the resiliency of youth leaving care.
  • Assisting youth in identifying goals and developing customized plans that encompasses tangible outcomes.
  • Helping them to identify, access, and navigate adult service systems relevant to their needs, as they transition to adulthood
  • Find and retain housing
