The Youth in Transition (YIT) Program provides culturally responsive, Afrocentric supports for Black youth aged 16–24 who are transitioning to adulthood, including youth involved with the Children’s Aid Society and those from the broader community. Through life skills development, workshops, mentorship, and team-building activities, the program promotes resilience, independence, and positive identity development.
The YIT team consists of a Youth in Transition Worker and a Housing Support Worker who work collaboratively to provide individualized, youth-centred support.
- Housing and or educational supports
- Employment and services training
- Life skills (e.g., financial management and household management)
- Help connecting with legal services and understanding your rights.
- Support accessing healthcare and mental health services.
- Referrals to trauma counselling or therapy, if needed.
